SC Department of Public Safety Case Manager in Richland County, South Carolina

Job Responsibilities

In this role, you will effectively coordinate Community Long Term Services to assigned participants in accordance with the SCDHHS/CLTC area offices.

You will provide CLTC case management services to CLTC participants as well as provide face to face contact with participant during visits. Case managers will follow up with the participant on a monthly basis.

You will develop and work collaboratively with other providers, public and private, and division within SCDMH/SCDHHS to provide clients with the most appropriate and effective care.

You will comply with federal, state, and third party payor regulations in the delivery of covered services ensuring these are appropriate and that documentation requirements are met as indicated by the SCDMH, SCDMH Corporate Compliance, and SCDHHS.

You will also perform other duties as required or assigned.

Minimum and Additional Requirements

State Requirements:Baccalaureate or graduate degree form an accredited college or university in a health or human services field that promotes the physical, psychosocial, and/or vocational well being of the individual being served. You must have documentation of at least two years of experience working with the target population.

Additional Comments

This position is located in the Midlands (Columbia) Area for CLTC. The Midlands area includes but not limited to Aiken, Allendale, Barnwell, Fairfield, Lexington, Newberry, Orangeburg, and Richland Counties. This position does require employee to travel.
