SC Department of Public Safety Clinical Care Coordinator/ Waccamaw in Horry County, South Carolina
In this position, you will effectively coordinate the overall care of patients in the Highway to Health grant mental health conditions ensuring appropriate and cost effective services.
Patients may involve individuals with mental illness, adults, children, to include those with physical, vocational, educational, social, and other needs in addition to mental health needs. Primary populations served will be rurally situated adults and children impacted by the lasting, devastating impact of Hurricane Florence.
You will participate in all grant required staffing/meetings to help establish a Comprehensive Needs Assessment identifying gaps in social determinants of health, and identifying/accessing needed services
You will develop comprehensive Case Management Plan (CMP) identifying specific goals and actions corresponding to the needs identified on the Comprehensive Needs Assessment (CNA).
You will follow up and monitor to ensure CMP is effectively implemented.
You will consistently adhere to the Office of Clinical Care Coordination productivity standards and grant standards.
You will properly render documents and bill all services allowable per Federal, State, CMS, HHS. third party payor, and DMH Corporate Compliance policies and procedures.
Minimum and Additional Requirements
_State Requirements: _A bachelor's degree and professional experience in a human services or social services program.
_Agency Requirements: _A bachelor's or graduate degree in a health or human services field that promotes the physical, psychosocial, and/or vocational well-being of the individual being served, and documentation of at least one year of case management experience working with the target population. The degree must be from an institution that is accredited by a nationally recognized educational accrediting body.

This position will serve patients of the Highway to Hope grant served by the Waccamaw MHC. You will be expected to travel within and outside of the MHC catchment where there is need for services.
