The Certified Nursing Assistant provides personal health care, and related services and additional tasks as assigned to the client in the home or facility; assists in providing a safe and hygienic environment, work cooperatively with patient, facility, family and share observations and problems with thesupervisor or RN who will report to IDG team as necessary. Works under the direction and supervision of a Registered Nurse and in accordance with the IDG care plan. May be required to work an on-call weekend rotation to meet the needs of patients 24/7.

  • $1,000 Sign on Bonus

  • Provides direct patient care to patients under the direction of the RN to ensure optimum care.

  • Demonstrates competency in the following areas including but not limited to: Measures temperatures, pulses, respirations and blood pressures; assists or performs bed bath, tub, shower or sponge bath; performs shampoo in bed, tub, shower or sink; checks, cleans and files nails; performs skin care; assists or performs oral hygiene; assists the patient with dressing; assists with toileting and elimination using toilet, bedside commode, bedpan, urinal or adult brief.

  • Provides personal care for each patient in accordance with the Hospice Aide Care Plan.

  • Prioritize daily needs of the patients and/or the request of the Registered Nurse.

  • Respect professional boundaries while providing care in facilities and patient homes.

  • Demonstrates safe techniques in all t ransfers and ambulation in the following area: Utilization of hoyer lift, wheelchair, and gait belt when transferring patients; safe and correct use of assistive devices including walker, cane and crutches.

  • Demonstrates proper body mechanics.

  • Describes the correct response to common emergency situations such as bites, burns, bleeding, poisoning, seizures, shock, diabetes, fire and weather emergencies.

  • Assists the patient with nutrition as directed on the plan of care, such as simple meal preparation and assisting with feeding and fluid intake.

  • Follows all environmental care and safety guidelines and standards per agency policy.

  • Introduces self, communicates in clear friendly manner, listens well and explains procedures prior to performing tasks.

  • Understands the relationship between the aide and jobs of other team members.

  • Flexible with handling assignment changes.

  • Works with a positive outlook/attitude and responds to suggestion and direction constructively.

  • Channels suggestions, criticism and questions to appropriate persons. Follows the chain of command.

  • Completes assignments in acceptable time limits according to performance guidelines or performance standards.

  • Avoid short cuts that may affect job quality.

  • In solving or applying skills consider the effects on patient/family & co-workers involved.

  • Shows initiative & takes on additional tasks when time & work load permits to assist others.

  • Offers assistance to co-workers and contributes with teamwork without emphasizing own personal goals.

  • Contributes to the management and efficient operation of the agency and demonstrates effective time management skills.

  • Maintains adequate supplies for each visit.

  • Accurate time, travel, and supplies are recorded of activity during visits.

  • Verifies assignment, reviews care plan and verifies with the patients and/or facilities the time of the visit before the visit occurs.

  • Able to complete tasks and work effectively without constant supervision.

  • Provides an environment which promotes respect for patient, privacy, and property.

  • Demonstrates knowledge of the patient’s Bill of Rights and upholds patient’s rights.

  • Provides necessary skills to appropriately report changes and document pertinent information and care rendered to the patient to ensure continuity of care.

  • Significant changes in the condition or environment of a patient are noted and reported to the supervising Registered Nurse in accordance with agency policy.

  • Recognizes and reports necessary revisions to the Certified Nursing Assistant plan of care within 24 hours.

  • Documents services provided and pertinent findings during each visit. Seeks clarification of duties when indicated.

  • Ensures signature includes first name, last name and credentials and is clearly legible on all visit notes.

  • Accurate time, travel, and supplies are recorded of activity during visits and completed by the end of each work day.

  • Practices accepted infection control principles.

  • Follows all infection control guidelines and standards per agency policy.

  • Must follow proper technique when utilizing clean/dirty bag technique.

  • Demonstrates commitment, professional growth and competency.

  • Attends the required 12 hours of in-services within the review year as assigned by Supervisor including annual safety update.

  • Written and verbal communication is expressed clearly and in a cooperative manner.

  • Participates on committees and task forces as requested.

  • Promotes the Agency philosophy, mission statement and administrative policies to ensure quality of care.

  • Maintains patient & staff privacy and confidentiality at all times pursuant to the HIPAA Privacy Final Rule.

  • Provides positive, supportive communication to physicians, patients, families, visitors, team members, facility members and other agency personnel.

  • Maintains availability by phone as directed by company policy.

  • Promotes Agency image by adhering to the dress code.

  • Reports to work, facilities and patient homes promptly as directed.

  • Handles the personal property of patients and their families with care.

  • Meets minimum productivity requirements based on agency standard.

  • Carries out the mission and values of Hospice.

  • Abides by company policy and procedures as outlined by company policy manual.

  • Performs other duties as assigned by the Supervisor.

Minimum Qualifications/Requirements:

EDUCATION: High School Diploma or equivalent required

LICENSURE: Must maintain certification as a Certified Nursing Assistant (C.N.A.) in the applicable state

EXPERIENCE: At least 1 year experience in the last 24 months as a Certified Nursing Assistant, a Home Health or Hospice Aide or a Nursing Assistant in a hospital or long term care facility

SKILLS: Must be able to read and write in English and follow written and verbal instructions in English effectively. Demonstrates interest in the welfare of the ill and desires to provide care in the home or facility. Ability to pass competency evaluation prior to performing care. Must be self motivated.

TRANSPORTATION: Reliable transportation

HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing.

Physical Requirements/Working Conditions:

  1. Complete required work on a daily basis.

  2. Cope with work involving ill, disabled, and dying patients.

  3. Sit, stand, bend, lift, and move intermittently. Must be able to lift at least 25lbs and bear the weight of an average adult effectively so as to be able to perform the above listed functions.

  4. Move equipment and supplies necessary for job functions.

  5. Read in a 12 point or larger type, speak and write English in order to fulfill job functions in an understandable way.

  6. See/hear or use prosthetics that enable accomplishment of job.

  7. Meet the health requirements of the agency.

  8. Possible exposure to infectious diseases.

  9. Abide by all occupational safety and bloodborne pathogen protocol to minimize illness and/or injury.

  10. Move, support and/or transfer patients as required to perform essential functions of position.
