This position is located in Beaufort, SC and under the direct supervision of the Area Coordinator. Coastal Empire CMHC is an approved site for the National Health Service Corps (NHSC), which offers student loan repayment of up to $30,000.00 to employees of NHSC - approved sites. You must be a US Citizen, you must be licensed to practice independently in the state where you are hired, and you must have unpaid government or commercial school loans. There is a two year service commitment required as well. *Job Purpose: *
Under the supervision of the Beaufort Clinic Director, provides outreach case management and crisis stabilization services to individuals who are at high risk for re-hospitalization.
- Job Functions:
1) Provides intensive outreach case management, treatment, and crisis stabilization services for seriously and persistently mentally ill adults who are at high risk for hospitalization.
2) In coordination with Hospital Diversion Coordinator and other team members, provides intensive transition services to enhance the natural support system requisite for maintaining community tenure for at-risk adults.
3) Ensures that documentation of all clinical services provided is in compliance with SCDMH Quality Assurance standards.
4) Participates in treatment team staffings, administrative meetings, and in-service training as directed.
Minimum and Additional Requirements
STATE REQUIREMENT: Bachelor's degree and professional experience in human services or social services program.
AGENCY REQUIREMENT: Master's degree in guidance, counseling or the social sciences; 1 year of experience in a clinical counseling program; and state licensed or licensed eligible.
Must be very computer literate. The ability to speak Spanish is a plus.
