Hospice Social Worker (MSW)



Toprovide social work services based on the individual needs of the patient andfamily as identified by the IDG and on the Psychosocial Assessment. Social Worker will providepsychosocial/supportive services to assist the patient and family by minimizingthe stress and problems that arise during the terminal illness, relatedconditions, and the dying process.


  • Providesprofessional counseling, assessment of psychosocial needs, assessments forfinancial needs and services including providing written explanation ofMedicare, Medicaid and Insurance benefits to prospective patients/families whennecessary, and provides guidance with regards to the grief and loss process.

  • Completespsychosocial history and assessment, assessing the needs of the patient andfamily systems’ psychosocial needs, within the guidelines of the policies andprocedures.

  • Providescrisis intervention and suicide assessment.

  • Participatesin the development of the plan of care with the interdisciplinary team,collaborates with other team members, and participates in updates to the planof care.

  • Providesservices to the patient and family consistent with the plan of care.

  • Assists thepatient and family to cope with the psychosocial changes and stresses relatedto the terminal illness.

  • Locates andobtains resources necessary to the financial, social, and emotional well-beingof the patient.

  • Providescounseling to family/caregivers to assist in coping with approaching death ofthe patient.

  • Documentsin the medical record all services.

  • Providesbereavement services to families of patients and refers families to otherresources as needed.

  • Providestraining in psycho-social needs of patients/families to hospice staff/nursinghome staff.

  • Recruit,train, and supervises hospice and home care volunteers, including assignmentsto families and patients.

  • Assist withthe physician and other health care team member’s in understanding thesignificant social and emotional factors related to the health problems of thepatient.

  • Supervise/adviseassigned BSW or baccalaureate degree person working in capacity of a socialworker.

  • Prepares clinicaland progress notes according to policy and procedures.

  • Usesappropriate community resources.

  • Participatesin discharge planning.

  • Participatesin ongoing inservice education.

  • Acts as aconsultant to agency personnel.

  • Coordinatescare provided to patients and families under the direction of the Director ofHealth Services.

  • Otherduties as assigned by Administrator.

  • Demonstrateknowledge of hospice services, progression of terminal illness, DNR/advancedirectives, financial resources, cultural diversity, family life cycle,grieving process, and burial planning.

  • Employskilled social work interventions (e.g. assessment, counseling, education,referral to community resources, crisis intervention).

  • Continuallyreassess psychosocial needs of patient/resident and family (includingbereavement risk).

  • Maintain,formulate and adhere to individualized care plans to addresspatient/resident/family needs.

  • Identifyand attempt to resolve ethical situations that arise in the course of providinghospice care.Seek assistance fromleadership as needed.


  • Participatein center/agency surveys (Licensure / JCAHO) and any subsequently requiredreports.

  • Attend andparticipate in continuing educational programs to keep abreast of changes inyour field as well as to maintain current license/certification, as required.

  • Attend andparticipate in mandatory inservices.

  • Honorpatients/residents’ rights to fair and equitable treatment, self-determination,individuality, privacy, property and civil rights, including the right to wagecomplaints.

  • Comply withcorporate compliance program.

  • Reportjob-related functions/tasks that involve occupational hazards includingexposure to blood and body fluids and others as necessary.

  • Followestablished safety regulations, to include fire protection & prevention,smoking regulations, infection control, etc.

  • Followestablished safety procedures when performing tasks and/or working withequipment.

  • Performother related duties as necessary and as directed by supervisor.




Master’sDegree in Social Work (MSW) required


One year experiencepreferred.


Masters level social workers who are eligiblefor state licensure/certification will successfully pass the licensure examwithin six months of employment with United Hospice.Social Workers will maintain currentlicensure and remain in good standing with the state licensure board.Social Workers will provide copies of theircurrent licenses to be filed in their personnel records.

CurrentDriver’s License and Automobile Liability Insurance.

ADDITIONAL QUALIFICATIONS: (Preferred qualifications)

  • Orientationand any other training that may be necessary to complete the assigned dutieseffectively.

  • Ability towork a flexible schedule.

  • Positivecriminal history check, Office of the Inspector General Sanction Check, andlicensure validations.

  • SocialWorkers will receive a Clinical Competency Assessment within the first 90 daysof employment and as indicated according to the Administrator’srecommendations.

  • Attendance– must maintain timely, regular attendance.

  • Punctuality.

  • Professionalappearance.

  • Communicateswell with patients/residents/clients and family members providing warm andfriendly greeting and an approachable attitude to families, visitors,patients/residents/clients and responds to expressed concerns while displayinga helpful, caring demeanor.Answers questionswhen appropriate in a professional manner.
